Medical Plan
The medical plans include medical, *vision and prescription coverage. Eligibility for regular full-time employees is on the first of the month following date of hire.
*when you enroll in the medical plan you are automatically enrolled in the VSP vision plan.
You have a choice of medical plan options based on where you reside in the United States. Employee contribution each pay period will differ based on plan choice and family size.
Exclusive Provider Organization (EPO Plan)
In order to have coverage under the Blue Cross EPO Plan, your health care provider must be contracted with Blue Cross or you must have been referred outside of the Blue Cross network by a contracted provider due to the lack of a specialist within the network. Out of network “Emergency care” is covered by the EPO Plan.
- You don’t need to choose a Primary Care Physician
- No referral is required to see a specialist.
- This medical plan option provides Blue Cross in network coverage only.
- Prescription drug plan.
Preferred Provider Organization (PPO Plan)
You have the option of utilizing a Blue Cross PPO provider or an Out-of-Network physician.
- You have the freedom to choose your health care provider each time you require health care.
- Out-of-Network benefits are typically paid at a lower level after meeting a deductible.
- Prescription drug plan.
HSA Compatible PPO Plan
- A comprehensive medical insurance plan that lets you obtain care from the providers of your choice.
- There is never a need to use a primary care physician or obtain authorizations for referrals to specialists.
- Annual out-of-pocket maximum limits to protect you from high costs.
- The plan deductible is waived for in-network preventive services.
With the HSA Compatible medical plan you get the protection of a medical plan; plus a Health Savings Account (HSA) feature which allows you to set aside pre-tax dollars to pay for eligible health care expenses.
- Your HSA deposits will earn interest, just like regular savings accounts.
- Your contributions are tax-free, and the earnings in your account grow tax-free.
- Withdrawals are tax-free, as long as they are used to pay for eligible health care expenses.
Dental Plan
Two dental plans are offered. Eligibility for full-time employees is on the first of the month following date of hire.
Delta Dental HMO Plan
- Co-pay for most diagnostic, preventative and restorative (fillings) services.
- Basic and Major services are covered and require a small co-payment.
- You must utilize a Delta HMO network dentist.
Delta Dental PPO Plan
- You have the option of utilizing an in-network dentist or any dentist of your choice.
- You have freedom to choose your dental provider each time you require dental care.
- Out-of-Network benefits for Basic services are covered lower rate after meeting the annual individual deductible. Major Services are covered at lower level after the annual deductible.
- The plan has a calendar year maximum per each individual on the plan.
Pet Assure
Pet Assure offers our employees who own pets, a significant savings towards the care of their furry friends. This is not an insurance program but a membership savings program for pet owners.
Assist America
Assist America is a worldwide emergency assistance service for traveling employees and their families. Assist America is an international assistance network that will instantly respond with the best possible solution to any medical emergency you may encounter. Assist America is staffed 24 hours a day, 365 days a year.
Life and Accidental Death & Dismemberment Insurance
- Company paid benefit provided through UNUM.
- You may also elect to purchase Voluntary Life Insurance coverage for yourself and your family and/or elect to purchase Voluntary AD&D Insurance.

Income Protection Plan
This benefit is available to all regular employees working 30 or more hours per week. This is a company provided benefit that will provides you with income protection in the event you are disabled and unable to work. The length of time you would be eligible to continue to receive your salary while you are disabled is based upon your length of service with the company.
Tuition Reimbursement
